To All Researchers that has been issued Certificate of Approval for Implementation

Please take note of the following POST APPROVAL DOCUMENTS,

Note: Forms can be accessed below.

     1. Progress Report

This document should be submitted six (6) months after your study approval (as stated in your Certificate of Approval) along with this please attached your approved Research Protocol (No Revisions Done).

Form:

Number of copies to be submitted: 2 hardcopies

      2. Final Report Form and Certification-

Final Report Form-This documents should be submitted after completion of your study (with results and discussion and conclusions and recommendations Chapter 4-5).

Certification: to prove that your complete/final study was presented to the department prior to submission to the Ethics Committee. (Note: Please have it reviewed first by your adviser before presenting it to the department)

Number of copies to be submitted: 2 hardcopies

     3. Request for Amendment-

This document must be submitted if you have revisions and changes after the study was approved in your Objectives, Methodology, Sample Size, Sampling Design, Participants (Inclusion and Exclusion), and Study Duration.

Number of copies to be submitted: 2 hardcopies

     4. All other pertinent documents should be submitted as needed or arises following the prescribe time as stated in your Notice of Approval for implementation.
